Life & Accidental Death Insurance
If you are authorized to work at least 15 hours per week, Roper St. Francis Healthcare provides you with Basic Term Life & Accidental Death and Dismemberment (AD&D) Benefits insured by Sun Life at no cost.
Basic Term Life and AD&D Insurance
- Teammates authorized to work at least 15 hours per week are eligible for Basic Term Life and AD&D insurance through Sun Life.
- Coverage is for 1 times your base annual earnings rounded to the next higher $1,000, up to a maximum of $2,500,000 (combined with any supplemental life/AD&D), at no cost to you.
- This benefit can be converted within 31 days of termination of employment subject to the terms of the contract.
- The effective date of this benefit is the first of the month following your date of hire or the date you become benefit-eligible due to a status change.
- If you are enrolled in the Basic Term Life Insurance plan, you will automatically be enrolled in Accidental Death & Dismemberment coverage through Sun Life at no additional cost to you.
- Your beneficiary will also receive an additional 25% of your AD&D benefit, up to a maximum of $25,000, if you are killed in an auto accident and the police report reveals you were wearing your seat belt at the time of the accident.
- The amount of your life insurance will be reduced by 65% on the date that you attain age 65 and 50% when you attain age 70. The reduced amount of coverage will be rounded to the next higher multiple of $1,000. An appropriate adjustment to your premium will also be made. Supplemental Term Life and AD&D Insurance
Teammates authorized to work at least 15 hours week may apply for an additional one to six times your base annual earnings in life insurance by electing Supplemental Life Insurance coverage through Sun Life. In addition, you will receive a matching amount of Accidental Death & Dismemberment coverage.
- The maximum coverage allowed is $2,500,000 (combined with Basic Life/AD&D) and the teammate pays the full cost of the Supplemental Insurance.
- Rates are age banded and range from $0.050 to $2.060 per month per $1,000 of coverage.
- The guaranteed issue amount for those who are newly eligible is two times your base annual earnings.
- Evidence of Insurability is required if you enroll outside your initial period of eligibility or if you are increasing your coverage.
- This coverage is portable within 31 days of termination of employment subject to the terms of the contract.
- The effective date of this benefit is the first of the month following your date of hire or the date you become benefit-eligible due to a status change.
- The amount of your life insurance will be reduced by 35% on the date that you attain age 65 and 50% when you attain age 70. The reduced amount of coverage will be rounded to the next higher multiple of $1,000. An appropriate adjustment to your premium will also be made. Spouse/Child Life and AD&D
Teammates authorized to work at least 15 hours per week may apply for Dependent Term Life Insurance through Sun Life.
- You may cover your spouse for up to $100,000 (in increments of $10,000) and dependent children for up to $15,000 (in increments of $5,000). The guaranteed issue for spouse life insurance is $30,000; anything greater will require evidence of insurability.
- You will receive a matching amount of Accidental Death & Dismemberment.
- This coverage is portable within 31 days of termination of employment subject to the terms of the contract.
The effective date of this benefit is the first of the month following your date of hire or the date you become benefit eligible due to a status change. If your spouse is also employed by Roper St. Francis Healthcare, you may not elect dependent spouse coverage. Additionally, only one parent may elect dependent child coverage when both parents are employed by Roper St. Francis Healthcare.
