If you are authorized to work at least 64 hours per pay period, Roper St. Francis Healthcare provides you with Basic Term Life & Accidental Death and Dismemberment (AD&D) Insurance underwritten by The Hartford at no cost. You may purchase additional coverage for you and your dependents at competitive rates.

2019 Universal Life Insurance

Please note the Whole Life insurance coverage will no longer be offered in 2019.  Instead, Universal Life Insurance will be offered through Transamerica.

Basic Term Life and AD&D Insurance
  • Teammates authorized to work 64 or more hours per pay period are eligible for Basic Term Life and AD&D insurance through The Hartford.
  • Coverage is for 1 times your base annual salary rounded to the next higher $1,000, up to a maximum of $125,000, at no cost to you.
  • This benefit can be converted within 31 days of termination of employment subject to the terms of the contract.
  • The effective date of this benefit is 90 days from your date of hire or the date you become benefit-eligible due to a status change.
  • If you are enrolled in the Basic Term Life Insurance plan, you will automatically be enrolled in Accidental Death & Dismemberment coverage through The Hartford at no additional cost to you.
  • Your beneficiary will also receive an additional 10% of your AD&D benefit, up to a maximum of $12,500, if you are killed in an auto accident and the police report reveals you were wearing your seat belt at the time of the accident.

Supplemental Term Life and AD&D Insurance

Teammates authorized to work 64 or more hours per pay period may apply for an additional one to four times your base annual salary in life insurance by electing Supplemental Life Insurance coverage through The Hartford. In addition, you will receive a matching amount of Accidental Death & Dismemberment coverage.

  • The maximum coverage allowed is $500,000 and the teammate pays the full cost.
  • Rates are age banded and range from $0.030 to $1.766 per month per $1,000 of coverage.
  • The guaranteed issue amount for those who are newly eligible is $500,000.
  • Evidence of Insurability is required if you enroll outside your initial period of eligibility or if you are increasing your coverage.
  • This coverage is portable within 31 days of termination of employment subject to the terms of the contract.
  • The effective date of this benefit is either 90 days from your date of hire or the date you become benefit-eligible due to a status change.

Dependent Term Life

Teammates authorized to work 64 or more hours per pay period may apply for Dependent Term Life Insurance through The Hartford.

 

 

 

  • You may cover your spouse for $10,000 and dependent children for $5,000.
  • You will receive a matching amount of Accidental Death & Dismemberment.
  • The cost is $0.90 per pay period regardless of the number of dependents covered.
  • This coverage is portable within 31 days of termination of employment subject to the terms of the contract.

The effective date of this benefit is either 90 days from your date of hire or the date you become benefit eligible due to a status change. If your spouse is also employed by Roper St. Francis Healthcare, you may not elect dependent spouse coverage. Additionally, only one parent may elect dependent child coverage when both parents are employed by Roper St. Francis Healthcare.

Whole Life

Roper St. Francis Healthcare teammates authorized to work at least 40 hours per pay period may enroll in Whole Life Insurance from Voya.

  • This is a traditional life insurance product with a fixed death benefit.
  • It builds cash value at a guaranteed rate and premiums are based on the death benefit and age.
  • Coverage is available for teammates, spouses, children and grandchildren and is portable if you leave Roper St. Francis.
